Epic Games added Survivor Medals with the new Fortnite season. The new feature can be seen in the quest tab and also in the game. What makes it even more special is the fact that you can unlock a new cosmetic item for free with the feature.

How to earn Survivor Medals in Fortnite Season 4
Survivor Medals are similar to Accolades, a feature first added in Fortnite Chapter 2. You earn them by simply staying alive and completing special quests. However, it’s important to note that Fortnite Survivor medals have seven rarities, ranging from Common (Gray) to Mythic (Gold).
If you stay alive long enough, you will receive your first medal. To upgrade it, you will need to complete a Survivor Quest which challenges you to stay alive while getting a certain number of eliminations. These quests are available in Battle Royale and Zero Build modes.
Survivor Quests get progressively harder, so earning the Mythic medal will be one of the toughest challenges in Fortnite Chapter 4 – Season 4.
