You can find Astarion is located southwest of the Roadside Cliffs, near the Nautiloid ship wreckage on the beach. If you travel across the wreckage toward the western side, you will likely hear Astarion calling out for help. However, when you first meet him outside the crash, he will try to slit your throat. Thankfully, with various charisma checks, you can break free from his hold.

After this, you can recruit Astarion to join your party. After some introductions, you will learn that Astarion has also been infected by the illithid parasites. You can then ask him to join your party; this will also activate Astarion companion quest, The Pale Elf.

If you have less than four extra companions at this point, he will automatically join your party. If your party is full he will travel to your place of long rest and wait until you dismiss another member and then ask him to join you.
