Should you remove the brain in Baldur’s Gate 3? There are many choices you will make throughout the game. Because of this, it helps to have characters who are proficient in all ability checks from Dexterity and Strength to Wisdom and Intelligence.

After leaving your Mind Flayer Pod, you will find an exposed brain that will talk to you. This creature will beg for you to remove it from the skull of the dead Thrall. Since you can’t magically teleport the brain out of the skull like a wizard, you will need to make a check to either remove or destroy it.

Later on, you can free Shadowheart from her prison, adding another member to your party. Even later in the game, you can get more characters like Wyll and Gale. Until then, you need to make this difficult choice. Should you remove or destroy the brain in Baldur’s Gate 3?

Baldur’s Gate 3: Should you remove or destroy the brain?

After having a Mind Flayer put a tadpole with teeth in your head and entering the second room of the Nautilid, you can go up a lift to find a dead Thrall attached to a neural apparatus. The brain will speak to you and ask to be removed. During this time, you have multiple choices:

  1. [Investigation] Insept the exposed brain
  2. [Strength] Break the Skull
  3. [Dexterity] Gently prise the brain from the skull

Here is what happens with each choice.

Inspect the Exposed Brain

This option will lead to you having another choice that requires a medicine check. With this choice, you can attempt a cerebral extraction, staying mindful of the swelling of the brain.

Afterward, this will lead to the choice of mutilating the brain or sparing it.

Break the Skull

Breaking the skull will result in a strength check. Afterward, you will be given the choice of mutilating the brain sparing it.

Gently Pris the Brain from the Skull

If you choose this option, you will need to make a dexterity DC of 10. On success, you will be given the choice of mutilating the brain or sparing it.

Should you mutilate or spare the brain?

If you choose to mutilate the brain, you will be forced to make a dexterity check. On a success, you the brain will become an Intellect Devourer and run away. On a failure, the brain will still become an Intellect Devourer but attack you instead.

If you choose to spare the brain, it will turn into an Intellect Devourer and then join you as a temporary ally while you are aboard the Nautiloid Ship. This is by far the best choice of all of them.

This little Intellect Devourer will help you escape the ship and can be used as an ally to defeat Command Zhalk.
