When Withers asks you this question a second time, you will receive six new questions. Depending on your Race, you could also receive a seventh question.

Within the dialogue above, you may also find a few selfish answers. However, if you choose answers one and four, the Withers will be satisfied with your response. Regardless of how you answer Wither’s question, it will not affect your playthrough. Once you have interacted with the undead NPC, he can still be found at your campsite. At the campsite, players can use the Withers’ respec ability and recruit Hirelings. 

You can find Withers in the Dank Crypt once you explore the Overgrown Ruins. Once you make your way to the Chapel Entrance east of the Roadside Cliffs, you can find him at the far end of the Crypt. Beat the various bandits and enter the locked door. Then, you will find him in a richly adorned Sarcophagus in Baldur’s Gate 3.
