There isn’t much downside to Volo removing the parasite and we think it is a good choice. After getting back to Druid Grove, Volo will offer to remove the tadpole infesting your brain. This is a surprisingly positive choice as it will help you later on in the game. Be wary, it also will hurt you. After all, Volo is more proficient in charisma than intelligence.
You will be faced with a choice on whether or not to allow Volo to perform the surgery. You will even be given the opportunity to back out multiple times before it is complete. After the surgery is done, your eyeball will be gone and the parasite will still be in your head.
While you will be missing your eye, there is a bonus to this. Volo will replace your eye with Volo’s Ersatz Eye. This will allow you to see invisible creatures around you during battle. There is a downside to having the ability to see hostile invisible forces. The cursed item comes with a decrease in your intelligence, AC, and saving throws. It’s up to you whether this tradeoff is worth it.
If your stats are high enough, this tradeoff isn’t very bad. However, certain classes like rogue, wizard, and cleric, will somewhat suffer from the effects of the eye. As long as you aren’t trying to be a thief in the near future, you should be okay to have the eye replaced. However, if you plan on the ways of thievery, we’d recommend not removing the eyeball.
Where is Volo in Baldur’s Gate 3?
Before you can rescue Volo, you will need to locate him in the Goblin’s camp during the quest that leads you over in that direction. He can be found outside and then later found in a cage inside the Shattered Sanctum temple. This is also the area where you can find goblin leaders Dror Ragzlin and Minthara and where you can rescue Halsin. There are many opponents in this fortress, so rescuing Volo isn’t the easiest task.
The eye is useful in fights like the Hag boss fight against Auntie Ethel as you will be able to see her while invisible. This makes attacking the Hag with melee weapons very easy.
